For accidents involving students or the general public, a PVCC Accident Report Form must be completed by the supervising faculty or staff member, or by the person involved, as appropriate. Reports of accidents involving students and the general public are submitted to the Vice President for Finance and Administrative Services, Room 241.
For accidents involving PVCC employees, including student employees, the employee must complete an Incident Report Form. Reports of accidents involving employees are submitted to Human Resources, Room 810.
